Rare ocular and skin lesions of Marginal Zone Lymphoma detected by F-FDG PET/CT

چکیده
We report a case of a 75-year old man that came at hematologist’s attention for lymphoma evaluation due to axillary lymph node enlargement and fever. Thorax, abdomen and pelvis Contrast-Enhanced Computed Tomography (CECT) showed lymph nodes, spleen and liver lesions. Axillary lymph node biopsy was performed and the diagnosis of marginal zone lymphoma (MZL) at stage IV was postulated, then the patient was submitted to chemotherapy (CHT) following the R-CVP scheme. After the end of the eighth cycle of CHT he was submitted to a restaging CECT that showed lymph nodes size reduction in all the sites identified on the staging exam. Furthermore the liver lesions disappeared and spleen lesions size was reduced. A whole-body and head F-FDG PET/CT was also performed that showed F-FDG uptake lesions in right axillary lymph nodes, spleen and liver and the identification of two more extranodal sites, respectively in conjunctiva and skin. Then the necessity of additional CHT cycles and radiotherapy on extranodal sites was postulated. The patient declined the new therapies and unfortunately succumbed four months later. In our case whole body and head F-FDG-PET/CT, finding two new extranodal lymphomatous sites and confirming the persistence of the disease, refined and guided the management of the patient suggesting the necessity of additional CHT cycles and radiotherapy on extranodal sites. After histopathological confirmation, a systemic work-up by an oncologist should include whole body and head F-FDGPET/CT to detect possible systemic involvement and guide specific following diagnostic exams.

متن کامل18F-FDG PET/CT in Neurolymphomatosis: Report of 3 Cases
Neurolymphomatosis is a rare manifestation of non-Hodgkin lymphoma characterized by infiltration of peripheral nerves, nerve roots, plexus and cranial nerves by malignant lymphocytes. This report presents positron emission tomography/computed tomography (PET/CT)imaging with 2-deoxy-2-18F-fluoro-D-glucose (18F-FDG) in 3 cases of non-Hodgkin lymphoma with nerve infiltration, including one newly d...
متن کاملF-18 FDG PET/CT Imaging of Eccrine Sweat Gland Carcinoma of the Scrotum with Extensive Regional and Distant Metastases
Eccrine carcinoma is an extremely rare malignant skin cancer arising from eccrine sweat glands with a high metastatic potential. It mainly occurs in the elderly, with equal incidence in both sexes. It usually spreads to regional lymph nodes, with liver, lungs, and bones being the most common sites of distant metastasis. Because of tumor rarity, little is known about the value of F18- FDG PET/CT...
متن کاملEarly detection of intravascular large B-cell lymphoma by 18FDG-PET/CT with diffuse FDG uptake in the lung without respiratory symptoms or chest CT abnormalities
Intravascular large B-cell lymphoma (IVLBCL) is a rare and aggressive subtype of systemic extranodal non-Hodgkin diffuse large B-cell lymphoma (DLBCL). We report a rare case of IVLBCL who showed diffuse 18F-fluorodeoxyglucose (FDG) uptake in the lung in FDG-positron emission tomography/computed tomography (PET/CT) without respiratory symptoms or chest CT abnormalities. Serum biochemical studies...
متن کاملRare skin and testicular extranodal involvement in a patient with marginal zone lymphoma on 18F-FDG PET-CT imaging
Hodgkin lymphoma (HL) and Non-Hodgkin lymphoma (NHL) are the malignant neoplasm spectrum originating from the lymphoid system and constituting 8% of all malignancies. Although, both are known as primary malignancies of lymph nodes, extranodal involvement can be seen. In this case presentation, in addition to nodal and bone involvement areas in a patient diagnosed as marginal zone lymphoma, rare...
